Our support coordination team will assist you to connect with mainstream, community – and funded support providers – to help you achieve your goals.
What you can expect from us
Our support coordination team has a range of backgrounds and experiences to ensure we are able to meet your goals and support you, no matter your circumstances.
We deliver this service through in-person meetings, over the phone or via video depending on your preferences.
There are two levels of support coordination that may be funded in your plan:
Level 2: Coordination of supports
Support to put in place a mix of supports to increase your capacity to maintain relationships, manage tasks, live more independently and be included in your community. We aim to build your confidence and skills to direct your life, not just your services.
Level 3: Specialist support coordination
A higher level of support for participants whose situations are more complex and who need specialist support. A specialist support coordinator will assist you to manage challenges in your support environment and ensure a consistent delivery of service.
Our support coordinators can assist with:
- Helping you understand your NDIS plan
- Planning and coordinating your supports
- Connecting you with community, mainstream and funded services
- Reporting to the NDIA on progress and barriers
- Assisting you to become more independent
- Preparing for unexpected events and changes
