Support Coordination

Support Coordination is designed to help you manage your support services a whole lot easier. Support Coordinators assist with finding the right providers, and helping you understand and navigate the National Disability Insurance Scheme (NDIS).

What will a Support Coordinator do

Work with you to understand and implement your NDIS plan funding. Making the most of your NDIS funding can be hard. As dedicated Specialist Support Coordinators, we work for you – managing Service Agreements, helping you to understand and learn more about the NDIS. Most importantly, we are here to help you live the life you choose.

Help connect you to services based on your goals. We will connect you with local providers including community groups and activities, therapists, and accommodation.

Negotiate with your providers and organise service and supports. We work with your service providers on your Service Agreement to outline the role and responsibilities of your supports. We work with you to monitor the services being provided to you to ensure that they meet your expectations and needs.

Help you prepare for plan reviews and gather reports from providers. We will help you prepare for your NDIS plan reassessment meetings and help you if there has been a change of circumstances which affects your goals and funding support.

How we work

Our Support Coordinators can assist you identify the services and service providers that best meet your needs, allowing you to exercise your choice and control. We work with you on your terms, as well as behind the scenes to assist you in reaching your individual goals.

Communicating and connecting on your terms.
Identifying and documenting your goals.
Work with you to ensure you have the confidence to achieve your goals, including:
-Helping you explore advocacy services if required
-Monitoring your progress towards your NDIS goals and the performance of the support services you use.

Find the best service provider and connections in your community that align to your goals, including:
-Connecting you with a strong support team and coordinate service providers and supports (including mainstream, informal, community and funded supports)
-Obtaining quotes for services and set up Service Agreements with providers
-Sourcing suitable Home and Living options
-Linking you into a job, or other community activities
-Planning and exploring options for times of key life transitions, such as leaving school or moving house

Complete the paperwork and administration associated with your plan including mandatory reporting.
-Reviewing your goals prior to your NDIS plan reassessment.
-Requesting a plan reassessment if your plan does not meet your needs
-Coordinate assessments, reports, and health and medical specialists

Implement and monitor your plan, ensuring you have the support required to achieve your goals, including: responding to a crisis or identify solutions for managing difficult situations.

What we offer

We offer a variety of support options including:
Level 2: Coordination of supports
Support to put in place a mix of supports to increase your capacity to maintain relationships, manage tasks, live more independently and be included in your community. We aim to build your confidence and skills to direct your life, not just your services.

Level 3: Specialist support coordination
A higher level of support for participants whose situations are more complex and who need specialist support. A specialist Support Coordinator will assist you to manage challenges in your support environment and ensure a consistent delivery of service.
